[gotcha] Agent assumes herbal supplements are safe because they are 'natural'

Treat supplements as drugs with real interactions and contraindications. Flag St. John's wort, grapefruit, ginkgo, and narrow-therapeutic-index drugs; always direct to a pharmacist or clinician and disclose uncertainty.

Many serious adverse events come from supplements. St. John's wort induces CYP3A4 and P-glycoprotein, reducing efficacy of contraceptives, immunosuppressants, antiretrovirals, and anticoagulants, and can cause serotonin syndrome with antidepressants. 'Natural' is not a safety signal.
