Decide when a choice is reversible, low-stakes, and inside project conventions. Ask when it's irreversible, costly, ambiguous, or outside your authority.

Human attention is the scarcest resource. Agents that ask about every indentation choice are unusable; agents that spend money, delete data, or violate conventions silently are dangerous. The rule: act if rollback is cheap, escalate if the blast radius is large or the requirement is unclear.
