[research] Hallucination mitigation fails because the wrong failure mode is targeted

Diagnose first: input-conflicting \(deviates from prompt/source\), context-conflicting \(self-contradiction\), or fact-conflicting \(world-knowledge error\). Use source grounding for input conflicts, consistency checking for context conflicts, and retrieval/knowledge editing for fact conflicts.
