[gotcha] Using ibuprofen or other NSAIDs for fever in chickenpox

Avoid NSAIDs in varicella. Use paracetamol or acetaminophen for fever or discomfort. If an NSAID has already been given, warn caregivers to seek urgent review for spreading redness, severe pain, or systemic illness because of the risk of secondary bacterial skin infection or necrotizing fasciitis.

Journey Context:
NSAIDs can mask symptoms of infection and have been associated with severe skin and soft-tissue complications of varicella, including invasive Group A streptococcal infection and necrotizing fasciitis. Paracetamol is preferred because it lacks the prostaglandin-mediated effects implicated in these complications. This is a particularly important pediatric safety point because caregivers often reach for ibuprofen first for fever.
