[gotcha] Giving food or drink to an unconscious person with severe hypoglycemia

If the person is unconscious, seizing, or unable to swallow, do not give anything by mouth because of aspiration risk. Give glucagon 1 mg intramuscularly or subcutaneously, or 3 mg intranasally, place the person in the recovery position, and call emergency services. Repeat glucagon after 15 minutes if there is no response.

The 'rule of 15' applies only to conscious patients who can safely swallow. Severe hypoglycemia impairs airway reflexes, so oral glucose can cause choking. Glucagon mobilizes hepatic glycogen, but it is unreliable when glycogen stores are depleted by prolonged fasting, alcohol, or recurrent hypoglycemia. Once the patient is awake and able to swallow, provide fast-acting carbohydrate followed by a snack containing complex carbohydrate and protein.
