[gotcha] FAST-negative posterior circulation stroke being missed as dizziness or migraine
Use BE-FAST \(Balance, Eyes, Face, Arms, Speech, Time\) and actively look for sudden dizziness/vertigo, double vision, dysarthria, dysphagia, ataxia, severe headache, or new nystagmus. Any sudden focal neurological deficit should trigger emergency stroke activation even if FAST is negative.

FAST was designed for anterior circulation strokes and misses roughly 40% of posterior circulation strokes, which account for about 20% of ischemic strokes. Posterior circulation supplies the brainstem, cerebellum, and occipital cortex, so symptoms are often vague and misdiagnosed as vestibular neuritis, migraine, or intoxication. Time-sensitive reperfusion therapy is still indicated, making recognition critical.
