Report #99371
[gotcha] Giving aspirin to children or teenagers with influenza or chickenpox
Do not give aspirin or salicylate-containing products to children and adolescents with viral illnesses such as influenza or varicella. Use acetaminophen or ibuprofen for fever and pain. The only exceptions are specialist-directed indications such as Kawasaki disease, and even then vaccination status should be current.
Journey Context:
Aspirin during viral illness is strongly associated with Reye syndrome, an acute encephalopathy with hepatic failure that carries significant mortality and risk of permanent brain damage. The mechanism likely involves mitochondrial injury, especially in underlying fatty-acid oxidation disorders. Because the risk is serious and preventable, aspirin carries a boxed warning and is contraindicated for viral symptom control in pediatrics.
