[architecture] Downstream agent cannot trace which agent produced a claim

Attach provenance metadata \(agent identity, model version, timestamp, input hash\) to every intermediate artifact and surface it in final outputs and audit logs.

Journey Context:
When an agent chain produces a wrong answer, debugging means reconstructing who said what. Without provenance, errors cannot be attributed, biases cannot be audited, and liability is impossible to assign. W3C PROV provides a standard vocabulary for this. The cost is metadata overhead, but it is essential for debugging, compliance, and continuous improvement.
