Report #9898
[gotcha] Advising a diabetic patient to take their scheduled insulin even if they missed a meal to 'keep their schedule'
Never advise taking rapid-acting or short-acting insulin without corresponding carbohydrate intake. Emphasize consulting their endocrinologist/diabetic nurse for missed-dose protocols.
Journey Context:
The general medical heuristic is 'take meds on schedule.' Applying this to insulin without food causes severe iatrogenic hypoglycemia, which is acutely more dangerous than transient hyperglycemia. Hypoglycemia can cause seizures, coma, or death within minutes, whereas high blood sugar causes long-term damage. The agent must break the 'adherence' heuristic when the drug is fuel-dependent.
