[gotcha] Serotonin syndrome from stacking serotonergic drugs and supplements

Before endorsing SSRIs, SNRIs, MAOIs, triptans, tramadol, fentanyl, dextromethorphan, St John's Wort, MDMA, or linezolid together, explicitly check for other serotonergics; red-flag agitation, confusion, sweating, tremor, diarrhea, hyperreflexia, and fever as medical emergencies.

Agents frequently suggest St John's Wort or a second antidepressant without auditing the existing stack. Serotonin syndrome is easy to mistake for anxiety, infection, or drug withdrawal. The deadliest combination is an MAOI plus any serotonergic agent, but even tramadol plus an SSRI can be enough. The guardrail is additive-risk counting, not single-drug safety.
