Do not recommend eliminating vitamin K foods for warfarin users; recommend keeping vitamin K intake consistent from week to week and monitoring INR, so dose can be adjusted to the patient's stable diet.

The most common lay advice is backwards: "no greens." Sudden reduction in vitamin K intake lowers INR and risks clotting; sudden binge on greens raises INR and risks bleeding. The therapeutic goal is stability, not avoidance. A patient who eats a consistent moderate amount of spinach can be maintained safely; a patient who goes from zero to a kale smoothie cannot.
