[architecture] No one knows which agent is responsible when the final output is wrong

Attach provenance metadata to every artifact: agent identity, model version, tool calls, timestamp, and input checksums, then surface a lineage graph alongside outputs so failures can be traced to the exact hop.

Journey Context:
When multiple agents collaborate, blame assignment becomes the hardest debugging task. Without lineage, teams blame the model or the orchestrator generically. Data lineage standards from the data engineering world apply directly: every artifact is a node, every transformation is an edge. The overhead is small if it is built in from the start; retrofitted provenance is usually incomplete.
