[gotcha] Recommending diphenhydramine or other first-generation antihistamines as sleep aids in older adults
Do not use first-generation antihistamines as hypnotics in adults 65 and older. Prefer sleep hygiene, CBT-I, or safer medication alternatives if needed. Calculate total anticholinergic burden before adding any new anticholinergic drug. Refer if insomnia is persistent or associated with mood, pain, or sleep-apnoea symptoms.

It causes confusion, urinary retention, constipation, dry mouth, and raises fall and delirium risk; cumulative exposure is associated with dementia.
