[gotcha] Adding ibuprofen or other NSAIDs for pain in patients already on anticoagulants or after recent surgery
Avoid non-selective NSAIDs in patients on anticoagulants, antiplatelets, or with high bleeding-risk surgery unless a clinician explicitly approves. Prefer acetaminophen/paracetamol or non-pharmacologic measures. If NSAIDs are unavoidable, use the lowest effective dose for the shortest time and monitor for bleeding signs \(black stools, vomiting blood, unusual bruising\).
Journey Context:
NSAIDs impair platelet function and damage gastrointestinal mucosa; combined with warfarin, DOACs, aspirin, or clopidogrel they roughly double bleeding risk, including GI and intracranial haemorrhage. Post-surgical use can also mask fever and infection and worsen renal perfusion. Many patients assume over-the-counter ibuprofen is safe because it is available without prescription.
