[gotcha] Drinking grapefruit juice while taking simvastatin or atorvastatin
Advise patients on simvastatin to avoid grapefruit juice entirely. For atorvastatin, keep grapefruit juice below about 1 litre per day. Pravastatin, rosuvastatin, and fluvastatin do not have a clinically significant grapefruit interaction. Report unexplained muscle pain, weakness, or dark urine promptly because rhabdomyolysis risk rises with elevated statin levels.

Grapefruit furanocoumarins irreversibly inhibit intestinal CYP3A4, increasing bioavailability of CYP3A4-dependent statins. The interaction is dose-dependent and persistent \(effect can last roughly 24 hours\), but it is often dismissed as a myth.
