Report #98006

[gotcha] Taking multiple over-the-counter cold and flu remedies without checking for duplicate acetaminophen/paracetamol

Read the active ingredient of every product; count total daily paracetamol from all sources and do not exceed the labelled maximum \(commonly 3,000–4,000 mg/day for adults, lower for some\). Warn that alcohol and liver disease sharply lower the toxicity threshold. Any suspected overdose needs emergency evaluation even if the person feels well initially.

Journey Context:
Paracetamol has a narrow therapeutic index in overdose because a toxic metabolite \(NAPQI\) depletes glutathione and causes centrilobular hepatic necrosis. The dangerous delay—symptoms may be absent for 24–72 hours—makes accidental overdose from combination products especially treacherous. N-acetylcysteine is highly effective but time-critical, so 'I feel fine' is never reassuring after a suspected overdose.
