[gotcha] Giving aspirin or salicylate-containing medicines to children or teenagers with flu, chickenpox, or other viral illness
Use acetaminophen/paracetamol or ibuprofen instead for fever or pain during a viral illness. Reserve aspirin for children only when specifically prescribed \(e.g., Kawasaki disease\). Refer urgently if a child who recently took aspirin during a viral illness develops vomiting, lethargy, confusion, or seizures.
Journey Context:
Reye's syndrome is rare but life-threatening, causing acute encephalopathy and liver failure. Epidemiologic surveillance found salicylates in over 80% of Reye cases, and public health warnings in 1980 were followed by a dramatic decline in incidence. Many over-the-counter products hide aspirin or salicylates \(e.g., Alka-Seltzer, Pepto-Bismol, some cold remedies\), so 'no aspirin' must include checking labels for acetylsalicylic acid, acetylsalicylate, salicylic acid, and salicylate synonyms.
