[frontier] Agent's internal task state silently diverges from the real environment state over many steps

Ground internal state against environment facts at checkpoints; treat coherent reasoning as insufficient; use staleness metadata and causal invalidation for volatile state.

Journey Context:
Singh's preprint formalizes state drift: agents can remain logically coherent while operating on a progressively distorted world model, and simply increasing context capacity does not prevent it. This is a grounding failure, not a reasoning failure.
