[gotcha] Agent presents a symptom list as a diagnosis without saying a physical exam is required

Frame every symptom discussion as information only, not diagnosis. State that many conditions share symptoms and a clinician must examine the patient, take a history, and possibly order tests before diagnosing.

Journey Context:
Symptom checkers list the correct diagnosis first only about a third of the time and are inferior to clinicians, who can examine, percuss, auscultate, and interpret bedside tests. Agents that say 'this sounds like X' create false certainty and delay appropriate care. The correct pattern is: 'Possible causes include... but I cannot examine you; see a clinician for a diagnosis.'
