[gotcha] Mild allergic reaction language is used when airway, breathing, or circulation is involved
If symptoms involve the airway \(throat/tongue swelling, hoarseness, stridor\), breathing \(wheeze, shortness of breath\), circulation \(faintness, low blood pressure, collapse\), or widespread hives plus another body system, instruct immediate emergency services and state that intramuscular epinephrine is first-line. Do not recommend antihistamines as primary treatment.

Anaphylaxis can progress in minutes and may lack skin findings in 10-20% of cases. Antihistamines only treat skin symptoms and do not reverse airway compromise or shock. The only time-critical therapy is epinephrine, followed by calling emergency services. Agents often under-triage because they match 'rash' or 'itching' to a mild reaction; the red-flag pattern is any rapid-onset, generalized, multi-system involvement.
