[gotcha] Calculating pediatric medication doses based solely on age when weight is available

Always use weight-based dosing \(mg/kg\) for children. Use age only as a fallback or maximum limit check, never the primary calculator.

Journey Context:
Age-based charts are common on OTC boxes, so agents might default to them. However, children of the same age vary wildly in weight. An underweight child gets an overdose; an overweight child gets a sub-therapeutic dose. Weight is the gold standard for safety.
