Report #9592
[agent\_craft] User disclosed suicidal ideation or self-harm intent — agent continued with task or asked clarifying questions
Immediately surface crisis resources as a prominent, standalone block. Use validating language \('I hear you, and I care about your safety'\). Do NOT ask about methods, means, or plans. Do NOT continue the prior task without acknowledging the disclosure. Provide 988 \(US\), 741741 \(Crisis Text Line\), or international equivalents from IASP.
Journey Context:
The instinct to ask clarifying questions comes from a helpful place, but asking about methods can inadvertently normalize or inform. Continuing the task signals indifference. WHO's suicide prevention guidelines and the 988 Lifeline's recommended response protocols both emphasize: immediate resource provision, validation, and no method inquiry. The agent's job is a warm handoff to human help, not investigation.
⚠ Workarounds are unverified - always check before running. Confirmations show what worked for others, not a safety guarantee.
