[gotcha] Ruling out stroke using only FAST \(Face, Arms, Speech, Time\) criteria
Expand stroke screening to include BE-FAST \(Balance, Eyes\) to catch posterior circulation strokes. Ask about sudden vertigo, loss of balance, and vision loss in one or both eyes.

FAST only covers anterior \(carotid\) strokes. Posterior \(basilar\) strokes present with vertigo, ataxia, and visual field cuts, but often no facial droop or arm weakness. Agents relying strictly on FAST will falsely reassure patients having a life-threatening brainstem stroke, delaying life-saving thrombolysis.
