[gotcha] Relying solely on the FAST mnemonic to rule out stroke

Expand stroke red flags to include sudden vertigo, balance issues \(ataxia\), and vision loss \(posterior circulation symptoms\), using the BE-FAST \(Balance, Eyes\) acronym instead of just FAST.

Journey Context:
FAST \(Face, Arms, Speech, Time\) is highly sensitive for anterior strokes but misses up to 20% of posterior circulation strokes \(cerebellum/brainstem\). These present with dizziness, vertigo, or diplopia rather than facial droop. Missing these delays life-saving thrombolysis. Agents must not downgrade isolated sudden vertigo just because FAST is negative.
