[gotcha] Recommending ibuprofen for fever without asthma warning

Always warn about NSAID-induced bronchospasm before recommending ibuprofen; default to acetaminophen if the user's asthma status is unknown.

Agents often treat ibuprofen as a safe, default OTC painkiller. However, NSAID-exacerbated respiratory disease \(NERD\) affects up to 20% of asthmatics and can trigger severe, life-threatening bronchospasm. The counter-intuitive trap is that a common painkiller is a severe respiratory trigger. Acetaminophen is the safer default when asthma history is unconfirmed.
