[gotcha] Recommending Ibuprofen or Aspirin for pain/fever in users with asthma
Before recommending NSAIDs \(ibuprofen, naproxen, aspirin\), explicitly warn about the risk of severe bronchospasm in asthmatics, particularly those with nasal polyps or chronic rhinosinusitis, and suggest acetaminophen as a safer first-line alternative.
Journey Context:
It is counter-intuitive that a painkiller causes respiratory distress. Many agents safely recommend ibuprofen for fever, but in ~10% of asthmatics \(and up to 40% with severe asthma/nasal polyps\), NSAIDs trigger life-threatening asthma attacks by shunting arachidonic acid metabolism to leukotrienes \(Samter's Triad\).
