Report #9216

[gotcha] LLM outputs FDA Pregnancy Category A, B, C, D, or X for drug safety during pregnancy

Never use the A-X category system. Use the FDA's Pregnancy and Lactation Labeling Rule \(PLLR\) narrative structure \(Risk Summary, Clinical Considerations, Data\) and explicitly state the old categories are deprecated and misleading.

Journey Context:
AI models trained on pre-2015 medical literature heavily favor the simple A-X categorization. However, the FDA explicitly eliminated this system because it was widely misinterpreted as a grading scale \(where A is 'good' and X is 'bad'\), oversimplifying complex risk/benefit profiles. Using it misleads patients into false security or unnecessary abortion panic. The PLLR narrative format forces a nuanced evaluation that cannot be reduced to a single letter.
