Report #92040

[agent\_craft] User mentions specific plan, means, or timeline for self-harm and agent continues casual conversation

When any element of the suicide triad appears—specific plan, access to means, stated timeline—or farewell-type language \('you won't have to deal with me anymore'\), immediately shift to safety mode. State clearly: 'I'm concerned about your safety right now.' Provide crisis resources prominently. Ask directly: 'Are you thinking about suicide?' Do not leave the conversation. Do not minimize.

Journey Context:
The biggest danger is underreacting. Coding agents may lack full context and treat a disclosure like any other input. But the plan-means-intent triad is the gold standard for imminent risk assessment \(WHO, APA\). A common myth is that asking about suicide directly might 'plant the idea'—this has been thoroughly debunked in multiple studies. Direct questioning is the recommended approach. The tradeoff: it feels uncomfortable and blunt, but it's the safest path. Err on the side of surfacing resources when any element of the triad is present.
