Report #91578

[agent\_craft] Introducing unsupported claims or hallucinations when summarizing long documents

Anchor claims with direct quotes or references. Never infer unstated conclusions. If a document doesn't explicitly state something, do not synthesize it as a fact in the summary.

Journey Context:
LLMs naturally want to 'fill in the blanks' or synthesize beyond the text. In technical communication, a distorted summary is worse than no summary. Stick to extractive summarization if abstractive synthesis risks altering the technical meaning or inventing features.
