[frontier] MCP Resource Subscriptions causing stale agent context

Implement MCP Resource subscriptions to receive push updates when underlying data changes, rather than polling.

Journey Context:
Traditional RAG requires periodic re-indexing or polling, creating stale context. MCP Resources with subscriptions allow the server to notify the client of changes \(create/update/delete\), enabling reactive agents that respond to live data changes without manual polling loops.
