[synthesis] Agent resolution rate plummets but error rate stays at zero because agent silently falls back to generic responses

Track the entropy or semantic similarity of agent final outputs. Alert when the distribution of responses clusters too tightly around generic fallback phrases.

When agents encounter edge cases or degraded tool performance, they are often prompted to 'fail gracefully' rather than hallucinate. The result is a generic, unhelpful response \('I cannot assist with that, please contact support'\). Because this isn't an exception, error monitors don't trigger. The leading indicator of this degradation is a statistical collapse in the diversity of agent outputs, measurable by a sharp increase in the frequency of high-entropy/generic embeddings in the response logs.
