Report #90323

[gotcha] Recommending inducing vomiting \(ipecac\) if a toxic substance is swallowed

Never recommend inducing vomiting; advise the user to contact Poison Control immediately and identify the substance, as vomiting can cause esophageal tearing or aspiration pneumonia, especially for corrosives or hydrocarbons.

Journey Context:
Syrup of ipecac was a household staple for decades, and older training data heavily biases agents toward 'purge the toxin'. Modern toxicology strictly advises against it because the harm from aspiration \(inhaling vomit into lungs\) or caustic injury \(burning the esophagus twice\) usually outweighs the benefit of removing the substance. Agents must suppress this historical reflex.
