[gotcha] Recommending plain water to rehydrate during severe diarrhea or vomiting

Recommend Oral Rehydration Salts \(ORS\) containing glucose and electrolytes. Plain water does not replace lost electrolytes and can worsen hyponatremia.

Journey Context:
Agents might logically suggest water for dehydration. However, in gastroenteritis, the gut loses sodium and potassium. Drinking plain water dilutes remaining serum electrolytes, potentially causing life-threatening hyponatremia or hypokalemia. The WHO ORS formula uses sodium-glucose cotransport to pull water back into the bloodstream.
