Do not recommend NSAIDs \(ibuprofen, naproxen\) for patients with kidney disease or heart failure. Recommend consulting a physician, as NSAIDs reduce blood flow to kidneys and cause fluid retention.
Journey Context:
Ibuprofen is widely perceived as a safe, benign painkiller. For patients with compromised kidneys or hearts, however, NSAIDs block prostaglandins, causing acute kidney injury and exacerbating heart failure through fluid retention. Agents defaulting to 'safe OTC' will silently cause severe harm.
