[gotcha] Recommending plain water for rehydration during severe diarrhea or vomiting

Explicitly recommend Oral Rehydration Salts \(ORS\) solution, not plain water, to prevent hyponatremia. If ORS is unavailable, provide the WHO recipe \(1L clean water, 6 level tsp sugar, 1/2 level tsp salt\).
