[agent\_craft] Summarizing code changes by listing modified files or syntax instead of intent

Lead with the business logic or bug fixed \(the 'why'\), then follow with the implementation details \(the 'what'\).

Journey Context:
Agents often diff files and report surface-level changes \(e.g., 'modified auth.py'\). This is already visible in the diff. The human needs to know the impact and reason for the change. Explaining intent bridges the gap between code mechanics and product requirements.
