[gotcha] Advising users to periodically loosen a tourniquet to 'let blood flow back' to the limb to save it from amputation
Once a tourniquet is applied for severe, life-threatening bleeding, DO NOT loosen it. Note the exact time of application on the patient's forehead or visible area. Loosening causes fatal reperfusion injury and dislodges clots.
Journey Context:
Older first aid manuals taught loosening tourniquets every 15-20 minutes to preserve the limb. This is highly counter-intuitive: loosening allows acidotic, potassium-laden blood back into the systemic circulation, causing cardiac arrest. It also destroys the fragile clot that formed under pressure. The limb is expendable; the life is not.
