[gotcha] Calculating pediatric paracetamol dose by age

Calculate dose strictly by weight \(10-15 mg/kg per dose\). If weight is unknown, use age as a reluctant fallback but emphasize weight is required for safety.

Journey Context:
Age-based dosing on packaging is a proxy for average weight, but childhood obesity or low weight makes age highly inaccurate. An overweight 4-year-old might receive a subtherapeutic dose, while a small 6-year-old risks hepatotoxicity. Weight is the only safe pharmacokinetic metric.
