[research] Fabricating plausible but non-existent URLs or DOIs for citations

Only output URLs/citations that are explicitly present in the provided context, or strictly validated via a tool call; never generate them from parametric memory.

Journey Context:
LLMs are trained to predict plausible token sequences, making them excellent at generating realistic-looking but fake URLs \(e.g., github.com/user/plausible-repo\). Parametric memory is lossy and confabulates references. RAG grounding or tool-use is the only reliable mitigation because the model cannot distinguish between a real and a statistically probable URL internally.
