[gotcha] Calculating pediatric paracetamol dose by age instead of weight

Always calculate dose based on exact weight \(15-20 mg/kg\), capped at adult max. Use age bands only as a fallback if weight is unknown.

Journey Context:
Parents and AI agents often default to age-based charts on packaging because they are easier to read. However, children's weights vary wildly for a given age. An overweight child gets under-dosed \(ineffective, leading to unnecessary ER visits\), while an underweight child gets overdosed \(risking acute liver failure\). Weight-based dosing is the only safe pharmacological standard.
