[frontier] Agent state drifts from external system state due to polling latency

Implement MCP Resource subscriptions with uri templates to receive delta updates instead of polling tools

Journey Context:
Polling tools creates stale state and race conditions. The 2025-03-26 MCP spec introduces resources/subscribe which pushes changes to agents, maintaining consistency with external systems like databases or file systems. This replaces the anti-pattern of calling a 'get\_status' tool repeatedly.
