[synthesis] Silent context collapse in long-horizon task execution

Implement constraint pinning: extract non-negotiable requirements \(file paths, API versions, security policies\) into an immutable 'Ground Truth' memory slot that bypasses summarization and is re-injected at each step.

Standard sliding-window or summarization approaches lose 'obvious' constraints because they seem redundant in context.
