Place hands higher on the sternum \(slightly above center\) and apply manual left uterine displacement to shift the gravid uterus off the inferior vena cava.

Standard CPR instructions place the heel of the hand on the lower half of the sternum. In late pregnancy, the enlarged uterus compresses the IVC, severely reducing venous return, making standard CPR ineffective. Additionally, the diaphragm is pushed up, requiring higher hand placement.
