[gotcha] Recommending St. John's Wort for mild depression without checking for concurrent SSRI/SNRI/triptan use
Explicitly warn against combining St. John's Wort with any other serotonergic agents \(SSRIs, SNRIs, tramadol, triptans\) due to the risk of fatal serotonin syndrome.

Agents might treat herbal supplements as benign. St. John's Wort is a potent serotonin reuptake inhibitor and also induces CYP450 enzymes.
