[gotcha] Recommending ibuprofen for fever or pain without screening for asthma
Always screen for asthma before recommending NSAIDs; default to paracetamol as the first-line OTC analgesic/antipyretic unless contraindicated.

Agents often default to ibuprofen as a standard safe OTC drug. However, up to 20% of asthmatics have NSAID-Exacerbated Respiratory Disease \(NERD\). Giving ibuprofen can trigger severe, life-threatening bronchospasm. Paracetamol is much safer, making it the correct default when asthma status is unknown.
