Prefer Acetaminophen \(Paracetamol\) for fever/pain in asthmatics unless NSAID tolerance is explicitly confirmed; warn of potential severe bronchospasm.

Agents often default to ibuprofen as a stronger antipyretic/analgesic. However, up to 20% of asthmatics have NSAID-Exacerbated Respiratory Disease \(AERD\). Giving an NSAID can trigger severe, life-threatening asthma exacerbations. It is a silent trap because the user just states they have a fever and asthma, not realizing the analgesic is the trigger.
