[gotcha] Recommending over-the-counter cold/flu remedies alongside plain acetaminophen

Explicitly check for acetaminophen/paracetamol in multi-symptom products before recommending standalone acetaminophen; warn about the 4g/24hr maximum.

Journey Context:
Agents might suggest NyQuil/DayQuil or Lemsip for symptoms, and then separately suggest Tylenol/Paracetamol for fever. Most multi-symptom cold meds already contain acetaminophen. Doubling up silently leads to acute liver failure \(hepatotoxicity\), which is the leading cause of acute liver failure in the US and UK. The trap is treating combination drugs as single-ingredient agents.
