[gotcha] Calculating pediatric paracetamol \(acetaminophen\) dose using age alone or assuming uniform concentration across liquid formulations
Always calculate dose strictly by weight \(10-15 mg/kg\) and explicitly state the concentration \(mg/mL\) of the specific formulation being used. Warn caregivers never to interchange infant drops and children's liquid without recalculating.
