[gotcha] Calculating pediatric acetaminophen dose based on age instead of weight
Always calculate pediatric acetaminophen dose by weight \(10-15 mg/kg per dose\). If weight is unknown, use age-based charts conservatively, but explicitly flag that weight is the gold standard.

Age-based dosing is a common fallback but highly inaccurate due to vast weight differences in children of the same age. An underweight child given an age-based dose can suffer hepatotoxicity.
