Report #82090
[gotcha] Recommending antibiotics for a positive urine test in an asymptomatic elderly patient
Only recommend antibiotics for UTIs if there are specific localized genitourinary symptoms \(dysuria, frequency, urgency\). Do not treat asymptomatic bacteriuria, even in the elderly.
Journey Context:
Agents matching 'positive urine culture' to 'UTI -> antibiotics' miss a critical nuance. Asymptomatic bacteriuria is extremely common in older adults and does not cause confusion, falls, or lethargy \(which are often misattributed\). Treating it causes antibiotic resistance, C. diff infections, and side effects without any clinical benefit. The trap is treating the lab test, not the patient.
